- Following initial reception screen, if mental health issues are identified, complete a more in depth assessment, develop treatment/interventions/ strategies to support the individual’s mental health needs using the appropriate evidence based screening tools

- Work with the multidisciplinary team (MDT) to develop treatment /intervention options through the provision of supportive care plans

- Administer medications in accordance with DrPA agreed policies and guidance

Key Responsibilities & Duties
Medicines Reconciliation & Admission Screening
- Undertake comprehensive medicines reconciliation for new receptions and transfers, ensuring accurate and timely identification of current therapy, reducing medication errors and wastage
- Identify and resolve discrepancies, liaising with prescribers, community pharmacies, and external healthcare providers as needed
Medicines Optimisation & Patient Support
